| Administration
& Dosage |
- Bacterial conjunctivitis: (0.5% ophthalmic solution) day 1-2, 1-2 drops every 2 hr while awake (MAX 8 times/day); day 3-7, 1-2 drops every 4 hr while awake (MAX 4 times/day)
- Bacterial prostatitis, chronic: 500 mg IV or ORALLY every 24 hr for 28 days
- Bacterial sinusitis, acute: 750 mg IV or ORALLY every 24 hr for 5 days or 500 mg IV or ORALLY every 24 hr for 10-14 days
- Bronchitis, chronic, acute bacterial exacerbation: 500 mg IV or ORALLY every 24 hr for 7 days
- Chlamydial infection: 500 mg ORALLY once a day for 7 days
- Community acquired pneumonia: 500 mg IV or ORALLY every 24 hr for 7-14 days or 750 mg IV or ORALLY every 24 hr for 5 days
- Corneal ulcer: (1.5% ophthalmic solution) day 1-3, 1-2 drops every 30 min to 2 hr while awake and 4 hr and 6 hr after retiring; day 4 through treatment end, 1-2 drops every 1 to 4 hr while awake
- Epididymitis: (caused by enteric organisms or with negative gonococcal culture or nucleic acid amplification test) 500 mg ORALLY once daily for 10 days
- Infection of skin AND/OR subcutaneous tissue: (uncomplicated) 500 mg IV or ORALLY every 24 hr for 7-10 days
- Infection of skin AND/OR subcutaneous tissue: (complicated) 750 mg IV or ORALLY every 24 hr for 7-14 days
- Inhalational anthrax, Post-exposure; Prophylaxis: 500 mg IV or ORALLY every 24 hr for 60 days
- Nongonococcal urethritis: 500 mg ORALLY once daily for 7 days
- Nosocomial pneumonia: 750 mg IV or ORALLY every 24 hr for 7-14 days
- Pyelonephritis, acute (Mild to Moderate): 250 mg IV or ORALLY every 24 hr for 10 days
- Traveler's diarrhea: 500 mg ORALLY once daily for 1-3 days
- Urinary tract infectious disease: (uncomplicated) 250 mg IV or ORALLY every 24 hr for 3 days
- Urinary tract infectious disease: (complicated) 250 mg IV or ORALLY every 24 hr for 10 days
- safety and efficacy in children under 18 years of age have not been established (oral/IV)

- Neurologic: Headache (ophthalmic, 8% to 10% oral and intravenous, 5.8% ), Insomnia (4.6% )
- Ophthalmic: Blurred vision, Ophthalmic solution
- Cardiovascular: Prolonged QT interval, Torsades de pointes (rare ), Ventricular tachycardia (rare )
- Dermatologic: Stevens-Johnson syndrome (rare ), Toxic epidermal necrolysis (rare )
- Endocrine metabolic: Hypoglycemia (0.1% to 0.9% )
- Hematologic: Aplastic anemia (rare ), Hemolytic anemia (rare ), Pancytopenia (rare ), Thrombocytopenia (rare ), Thrombocytopenic purpura (rare )
- Hepatic: Hepatic necrosis (rare ), Hepatitis (rare ), Liver failure (0.1% to 0.9% )
- Immunologic: Anaphylactoid reaction (rare ), Immune hypersensitivity reaction (0.1% to 0.9% )
- Musculoskeletal: Traumatic or non-traumatic rupture of tendon (rare )
- Neurologic: Polyneuropathy (rare )
- Renal: Acute renal failure (0.1% to 0.9% )
- Respiratory: Extrinsic allergic alveolitis (rare )
- Other: Serum sickness due to drug (rare )
