Candidiasis
a) The usual oral dose for treatment of serious infections (septicemia, endocarditis, urinary tract infections, and pulmonary infections) caused by susceptible strains of candida is 50 to 150 milligrams/kilogram/day in 4 equally divided doses at 6 hour intervals. Capsules should be taken a few at a time over 15 minutes to minimize nausea and vomiting. The dose should be adjusted in patients with renal dysfunction. Except in urinary tract infections, flucytosine should be used in combination with amphotericin B for the treatment of systemic candidiasis because of rapid emergence of resistance to flucytosine in Candida isolates of patients receiving flucytosine alone (Prod Info Ancobon(R) capsules, 2003).
b) Oral doses of 250 milligrams/kilogram/day have been recommended in severe infections (Steer et al, 1972).
Cryptococcosis

Capsules should be taken a few at a time over 15 minutes to minimize nausea and vomiting. b)Doses of 150 milligrams/kilogram/day (37.5 milligrams/kilogram every 6 hours) orally in combination with amphotericin B have been recommended for the treatment of cryptococcal meningitis (Bayardelle et al, 1982; AMA Council on Drugs, 1986a; Mandell et al, 1985a). Flucytosine is not recommended as monotherapy for the treatment of cryptococcosis (Hospenthal & Bennett, 1998).
c) The Medical Letter recommends intravenous amphotericin B 0.5 to 0.7 milligram/kilogram daily (with or without flucytosine 100 mg/kg/day in 4 divided doses orally; this agent is usually poorly tolerated in HIV infection) for the treatment of cryptococcosis in AIDS patients. The duration of therapy is determined by clinical and serological response, which usually lasts at least 2 weeks (Anon, 1997).
