Adult
Induction of Anesthesia: IV, over 20-30 sec and allowing 2 mins for effect
Unpremedicated patients< 55 yrs:0.3-0.35 mg/kg (Total max. dose 0.6 mg/kg).
Unpremedicated patients ≧55 yrs:0.3 mg/kg.
Unpremedicated patients with severe systemic disease or other debilitation:0.15-0.25 mg/kg.
Premedicated patients< 55 yrs: 0.25 mg/kg.
Premedicated good risk surgical patients 55 yrs: 0.2 mg/kg.
Premedicated patients with severe systemic disease or debilitation:0.15 mg/kg.
Maintenance dose:incremental injections of approximately 25% of the induction dose should be given in response to signs of lightening of anesthesia and repeated as necessary.
Preoperative sedate, anxiolysis, amnesia: IM,
Good risk patient<60 yrs: 0.07-0.08 mg/kg 1hr before surgery.
Patients with COPD, other higher risk surgical patients, patients ≧60 yrs, and patients who have received -concomitant narcotics or other CNS depressants: 0.02-0.05 mg/kg (approximately 2-3 mg).
Older patients when the anticipated intensity and duration of sedation is less critical: 1 mg.
Psychotic agitation: IM,5-15 mg at 15 mins intervals as needed.
Sedation: IV continuous infusion,
Loading dose:0.01-0.05 mg/kg (approximately 0.5-4.0 mg) may be given slowly or infused over several minutes. This dose may be repeated at 10-15 min intervals as needed.
Maintenance dose: 0.02-0.10 mg/kg/hr (1-7 mg/hr). Assess sedation at regular intervals and adjust the infusion rate up or down by 25-50% to assure adequate sedation. Decrease infusion rate by 10-25% every few hours to find the minimum effective infusion rate.
Sedation, anxiolysis, amnesia for procedures: IV,
Healthy adults< 60 yrs:1-2.5 mg IV titrated slowly (max. infusion rate 1.25 mg/min). Wait> 2 mins after each increment to fully evaluate the sedative effect. A total dose> 5 mg is not usually necessary.
Patients ≧60 yrs, and debilitated or chronically ill patients: 1-1.5 mg (max. infusion rate 0.75 mg/min). A total dose> 3.5 mg is usually not necessary.
Maintenance dose: may give 25% of the dose used to first reach the sedative endpoint.
Pediatric
Preprocedural sedation, anxiolysis: PO, 0.25-1.0 mg/kg given as a single dose (max. 20 mg).
Sedation, anxiolysis, amnesia (prior to and during procedures or prior to anesthesia):IV, over 2-3 minutes, wait an additional 2-3 mins to fully evaluate the sedative effect before initiating a procedure or repeating a dose
6 mo-5 yrs:initial dose 0.05-0.1 mg/kg, total dose<6 mg.
6-12 yrs: initial dose 0.025-0.05 mg/kg; total dose<10 mg.
12-16 yrs: dose as adults; total dose< 10 mg. |
