Adult
Inj.
Acute myocardial infarction - Pain: 2-10 mg IV over 4 - 5 min.
Analgesia for a mechanically ventilated patient, ICU:
0.07- 0.5 mg/kg/hr IV infusion; or 0.01- 0.15 mg/kg q1-2h IV.
Anesthesia; Adjunct: 4 - 10 mg IV over 4 - 6 min.
Premedication for anesthetic procedure:
5 -15 mg IV very slowly.
Cancer pain, Chronic pain, Moderate to severe pain,Postoperative pain :
2 - 10 mg IV over 4 - 5 min; or 5 -20 mg q4h SC as needed; or 0.8 - 10 mg/hr IV infusion;
or 5 mg EPIDURAL inj. in lumbar region, additional incremental doses of 1 - 2 mg at appropriate intervals may be necessary, Max. 10 mg per 24 hr;
or 3.5 - 7.5 mg/day continuous EPIDURAL microinfusion for opioid-naive patients and 4.5 - 10 mg/day continuous EPIDURAL microinfusion for opioid-tolerant patients.
Pulmonary edema, acute; Adjunct:
4 -10 mg IV over 4 - 6 min; or 5 - 20 mg q4h SC as needed.
Pediatric
Inj.
Analgesia for a mechanically ventilated patient, ICU:
0.01 - 0.03 mg/kg/hr IV infusion.
Cancer pain, Chronic pain, Moderate to severe pain, Postoperative pain :
0.1 - 0.2 mg/kg q4h SC, Max. 15 mg/dose;
or 0.05 - 0.1 mg/kg IV, Max. 10 mg/dose.
0.1 mg/kg q4-6h IV/SC for neonates,
Premedication for anesthetic procedure:
0.05 - 0.1 mg/kg IV very slowly; Max. 10 mg/dose.
