Severe Genital herpes infections, initial episode: 5 mg/kg q8h for 5 days.
Mucocutaneous herpes simplex (HSV-1 and HSV-2) infections, immunocompromised pts: 5 mg/kg q8h for 7 days.
Herpes simplex encephalitis: 10 mg/kg q8h for 10 days.
Herpes zoster, caused by varicella zoster virus, in immunocompromised pts:3 12 yrs, 10 mg/kg q8h for 7 days.
Max. 20 mg/kg q8h.
Pediatric
Severe Genital herpes infections, initial episode:
Infants and children<12 yrs: 250 mg/m2 q8h for 5 days.
Mucocutaneous herpes simplex (HSV-1 and HSV-2) infections, immunocompromised pt’s:
Infants and children<12 yrs: 10 mg/kg q8h for 7 days.
Herpes simplex infections, neonatal:
Infants (birth-3 mo),10 mg/kg q8h for 10 days.
Herpes simplex encephalitis: 3 mo-12 yrs, 20 mg/kg q8h for 10 days.
Herpes zoster, caused by varicella zoster virus, immunocompromised patients:<12 yrs, 20 mg/kg q8h for 7 days.
